3) To complete the form online - Fill in the blank fields directly on the online form.
4) Click the "submit" button to email the form directly to db Construction Group Inc.
• Or, print the form out and fill in the fields manually, then fax the form to 412.464.4611.
5) If you wish to print the Blue Prints below, please follow the directions below.
• download them to your computer (by clicking on them and saving them to your desktop).
• Then send them to your local Kinkos or any other printing facilities by attaching them to an
email and sending it.
Please be sure to contact your printer to discuss details and make them aware that you
are sending files over.
